However usually there is a team including doctors, nurses, social workers and chaplains, similar to the hospice team.

And someone from your hospice team will usually visit you for an hour or so one or more times a week.

The hospice teams include nurses, social workers, volunteers, medicine and equipment.

You may need the support of a hospice team.

Your doctor can continue to direct your care and work closely with you and the hospice team.

If you remain at home, the hospice team supports your family in their caregiving.

The hospice team is on call 24 hours a day, seven days a week.
